When someone dies, what do I need?

Dear Family,

When someone dies many families do not know what to do. Pettett Funeral Home is here to help you in completing all of the details of making funeral arrangements.

Our funeral home has made a check list of things that we possibly will need from you or your family when making funeral arrangements.

These are only a few items and our funeral home will discuss all the other details with your family; but if you have any question, please do not hesitate to call the funeral home at any time.

 Sincerely yours,

Teresa Basinger and Charles Ebersohl

___Social Security Number of the deceased_______________

____(if there is a surviving spouse, we will need his/her number also__________________)

___ Military Discharge, if deceased had been in the military. (DD214)

___ Full set of clothing for the deceased, including underclothing. (If the clothing needs to be taken to the cleaners, please bring the clothing to the funeral home, and we will take the clothing to the cleaners.)

__ Glasses ____ Dentures ____ Jewelry

___ Picture of the deceased for the newspaper. (There can be other people in the picture, because we will only scan the deceased, and you will receive your picture back intact.)

___ Our funeral home will help you write the obituary, but if you would like to list relatives and memberships, feel free to bring that with you when we make the funeral arrangements.

___ Life Insurance policies

These are some of the initial items our funeral home might need whilte making funeral arrangements.  There are other items that we will discuss with you and your family, and we will help in any way we possibly can; plus we will help think of things that you might not have thought about.
